Dr. Andy’s AdviceDrivers sometimes assume that any doctor or healthcare provider can complete a DOT Physical. That is not the case for drivers subject to FMCSA medical certification requirements. Your examiner must be properly certified and listed on the FMCSA National Registry. My job as a Certified Medical Examiner is to apply the current Federal standards fairly and consistently, while making the process as clear and efficient as possible for the driver. A DOT Medical Examiner is not simply a healthcare professional who performs a routine physical. For interstate commercial motor vehicle drivers subject to FMCSA medical qualification requirements, the examiner must meet specific Federal certification requirements and be listed on the National Registry of Certified Medical Examiners.
FMCSA created the National Registry to help ensure that Medical Examiners understand the physical and mental demands of commercial driving and can apply the Federal physical qualification standards appropriately. A Certified Medical Examiner evaluates whether a driver meets those standards and reports the examination results through the National Registry.

He performs DOT Medical Exams for commercial drivers who need an initial examination, renewal, or updated medical certification. He also provides REQUIRED DOT Physicals for those entering Truck Driving Schools.
FMCSA allows healthcare professionals who are licensed, certified, or registered under applicable State law to perform physical examinations to pursue Medical Examiner certification. This can include doctors of medicine, doctors of osteopathy, physician assistants, advanced practice nurses, doctors of chiropractic, and other qualified medical professionals authorized by State law.
Being licensed to perform physical examinations is only the starting point. To become an FMCSA Certified Medical Examiner, the healthcare professional must complete the required training, pass the National Registry Medical Examiner Certification Test, have the applicable professional credential validated, and be certified and listed on the National Registry before performing interstate CMV physical qualification examinations.
During a DOT Medical Exam, the DOT Medical Examiner reviews the driver’s health history, medications, and relevant medical documentation and performs the required physical qualification examination. This includes items such as blood pressure and pulse, vision, hearing, urinalysis, and examination of body systems relevant to safely operating a commercial motor vehicle.
The Medical Examiner is responsible for applying the applicable FMCSA standards to the individual driver. Depending on the findings, the examiner may determine that the driver is medically qualified, may issue a shorter certification period when appropriate monitoring is needed, or may need additional medical information before making a certification decision.
The purpose is not to look for reasons to fail a driver. The goal is to determine whether the driver meets the applicable Federal medical qualification standards while helping the driver understand what information or documentation may be needed. Need a same-day exam? Call at least two hours ahead to confirm availability. Please do not assume an unexpected walk-in can be accommodated because Dr. Andy may not be at the office when you arrive. Pre-booking online is the easiest option and immediately saves $30 when the appointment qualifies for the online pre-booking rate.
On-Site DOT Physicals are also available at places of employment, trucking fleet locations, truck stops, larger fueling stations, and other appropriate locations throughout Tennessee. Mobile and on-site services are $129, and the $30 office pre-booking discount does not apply. Same-Day or Next-Day On-Site Service may be available, especially in Middle Tennessee.
Is a DOT Medical Examiner the same as my regular doctor? Not necessarily. Your regular healthcare provider may be able to become a Certified Medical Examiner, but interstate CMV physical qualification examinations must be performed by a Medical Examiner who is certified by FMCSA and listed on the National Registry.
Can a chiropractor be a DOT Medical Examiner? Yes. FMCSA specifically includes doctors of chiropractic among the healthcare professionals who may qualify to become Certified Medical Examiners when they meet the applicable requirements.
Can I use any Certified Medical Examiner in the country? FMCSA allows interstate CMV drivers to search for Certified Medical Examiners by city, state, or ZIP code through the National Registry. This is helpful for truck drivers who are away from home when their certification is due.
Does the Medical Examiner decide how long my certification lasts? The examiner applies FMCSA requirements to your individual medical situation. A DOT medical certificate can generally be issued for up to 24 months, but a shorter period may be appropriate when a condition requires monitoring.
What happens if the examiner needs more information? Depending on your medical history or examination findings, additional documentation may be needed before a final qualification decision can be made. Bringing relevant records with you can help avoid unnecessary delays.
